Objectives:

  1. Participants will be able to identify eligible patients for participation in a Diabetes Prevention Program.

  2. Participants will be able to explain the benefits of participation in the Diabetes Prevention Program to patients at high-risk for developing diabetes.

  3. Participants will be able to direct high-risk patients to resources for more information on  local Diabetes Prevention programming.

  4. Participants will learn to use My Resource Connection as a way to connect patients with community resources they need to address barriers to care.

  5. Participants will practice navigating My Resource Connection in order to utilize it effectively. 


Educational Review Systems is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. This program is approved for 1 hour (0.1 CEUs) of continuing pharmacy education credit.  Proof of participation will be posted to your NABP CPE profile within 4 to 6 weeks to participants who have successfully completed the post-test.   Participants must participate in the entire presentation and complete the course evaluation to receive continuing pharmacy education credit.
