Pharmacists' Patient Care Process

Established by the Joint Commission of Pharmacy Practitioners (JCPP)

JCPP has released a document titled the, Pharmacists’ Patient Care Process with a composed article as well. It serves as guidance for consistent delivery of patient care for pharmacists in all practice settings. It was developed by a group of national pharmacy organizations that work under the direction of JCPP. Its goal is high quality, cost-effective and accessible health care for patients. The JCPP feels this goal is best achieved through team-based patient centered care. It describes how pharmacists collect, assess, plan, implement, and follow-up through monitoring and evaluation of patients as part of a health care team.
