The Kansas Pharmacists Association (KPhA) has partnered with the Kansas Department of Health and Environment's Immunization Program to serve as one of pharmacy's primary points of contact for information related to the state's pharmacy-based COVID-19 vaccine response. Complete the following form to sign up, participate, and receive updates from KPhA and the KDHE regarding pharmacy-based COVID-19 vaccinations. Please note that all pharmacies interested in providing COVID-19 vaccines will be required to complete the Centers for Disease Control and Prevention (CDC) COVID-19 Vaccination Program Provider Agreement. The Kansas Immunization Program is currently developing the electronic version of this form that will allow you to fill in the form and have your information directly forwarded to WebIZ as a COVID-19 vaccine provider. In the meantime, you can review the CDC agreement and prepare your information for when the Kansas form is live, HERE.
Here are the steps you need to complete TODAY:
- Sign up to participate in the Kansas Pharmacy-Based COVID-19 Vaccine Response.
- Review and collect the required information in the CDC COVID-19 Vaccination Program Provider Agreement for future use.
- Complete the electronic CDC COVID-19 Vaccination Program Provider Agreement (when it becomes available).
We will be sending out additional information outlining other steps you can take to prepare to be a COVID-19 Vaccine Response site.
